Do sugary drinks cause dementia? What the research actually says
No single drink causes dementia. But large studies do link a regular habit of sugar-sweetened drinks — soda, sweetened fruit drinks, sweetened milk drinks — to a higher risk of dementia, and the association is stronger for sugar you drink than sugar you eat. The effect is modest and observational, so it can't prove cause and effect, and the story for diet drinks is genuinely mixed. The honest, evidence-based takeaway isn't fear — it's a swap: replacing a daily sugary drink with water, coffee or unsweetened tea is one of the simplest diet changes that may support long-term brain health.
Why drinking sugar looks worse than eating it
The most striking finding in this area comes from the UK Biobank, which followed 186,622 adults for about ten years (1,498 developed dementia). Researchers separated sugar by its source — and the source mattered a lot:
- Free sugars from soda and fruit drinks were associated with higher dementia risk — a hazard ratio of about 1.34 (roughly a third higher) at higher intake levels.
- Sweetened milk-based drinks showed a similar signal (hazard ratio about 1.39).
- Free sugars from solid foods showed no significant association with dementia.
- Moderate natural juice intake wasn't linked to higher risk — and may even have been slightly protective.
Why would a can of cola look worse than the same sugar in food? The leading explanation is how fast the sugar arrives. Liquid sugar hits the bloodstream quickly, driving sharp spikes in blood glucose and insulin, without the fibre, protein or fat that slow digestion down. It's association, not proof — but it's a consistent, biologically plausible signal from a very large study.
"But I drink diet soda" — here's the honest picture
This is where the internet's scary headlines get ahead of the science.
An early, widely-cited study (the Framingham Heart Study, published in Stroke in 2017) found people who drank artificially-sweetened ("diet") beverages daily were about three times more likely to have a stroke or develop Alzheimer's over ten years. Alarming — until you look closer.
The nonprofit ADDF Cognitive Vitality points out the catch: 26% of the diet-soda drinkers already had type-2 diabetes, versus 9% of non-drinkers. Once the researchers controlled for a whole block of cardiometabolic factors together — diabetes, blood pressure, cholesterol, waist-to-hip ratio, existing heart disease and APOE-e4 status — the Alzheimer's association weakened and was no longer statistically significant.
But diabetes alone doesn't explain it away. When the authors re-ran the analysis with everyone who already had diabetes removed, daily diet-drink intake still predicted Alzheimer's dementia — if anything more strongly. They described diabetes as "a partial but not full mediator." The most likely reading is that people already at higher metabolic risk are more apt to switch to diet drinks, so the drink may be partly a marker of risk rather than a cause of it. That's called reverse causation, and it's a classic trap in nutrition studies. With only 63 Alzheimer's cases, this study simply couldn't separate the possibilities.
More recent research keeps the picture muddy. Some 2026 cohort studies found no meaningful difference in dementia rates between older adults who drank sugary or diet drinks and those who rarely did, while other work has tied diet drinks to faster brain ageing — with associations showing up mainly in adults under 60. The honest summary: the evidence on artificially-sweetened drinks is mixed and unresolved. Anyone telling you diet soda definitely causes dementia is overstating what we know.

- Best choices: water, plain sparkling water, black coffee and unsweetened tea. Coffee and tea carry their own modest brain-health signals in some studies, and none of them deliver a sugar spike.
- Limit: regular soda, sweetened fruit drinks, sweetened milk and coffee drinks, and energy drinks — the ones most consistently linked to higher risk.
- Diet drinks: fine as an occasional bridge if they help you cut sugar, but don't treat them as a "free" health drink — the goal is to trend toward water, not to swap one habit you're unsure about for another.
- Whole fruit over fruit drinks: the fibre changes everything. Moderate real juice looked neutral; sugary "fruit drinks" did not.
