Low vitamin B12 and memory: the reversible cause worth checking
Most of what you read about memory is about risk you can only nudge. This one is different. Low vitamin B12 is one of the genuinely reversible contributors to memory problems, brain fog and confusion — and finding out is a simple blood test. That's exactly why standard cognitive workups, including the American Academy of Neurology's long-standing guidance, include a B12 check when someone reports a change in thinking: it's one of the few things on the list that can be corrected. But here's the honest other half, and it's the part the supplement aisle skips: taking B12 does not appear to boost memory if you aren't deficient. The message is simple. Test, don't guess.
What B12 actually does in there
Vitamin B12 isn't a "brain booster" in the marketing sense — it's a piece of basic machinery. Your body uses it to build and maintain myelin, the fatty insulation wrapped around your nerves, and to keep the chemistry running that makes healthy red blood cells. When B12 runs short, that insulation degrades and signals travel less cleanly. Because the same shortfall affects nerves in your hands and feet and the wiring in your brain, low B12 tends to show up as an odd combination: fuzzy thinking alongside physical symptoms that seem unrelated. That's the reason a memory complaint and a tingling foot belong in the same conversation with your doctor.
Who's most at risk — and why it's usually not diet
The common assumption is that B12 deficiency is about not eating enough of the right food. Sometimes it is. Far more often, it's about absorption — the B12 is on the plate, but the body can't get it out. The groups worth knowing about:
- Adults over about 50. Stomach acid production declines with age, and acid is what releases protein-bound B12 from food. This is common enough that the NIH's Office of Dietary Supplements advises people over 50 to get most of their B12 from fortified foods or supplements, which the body absorbs more easily.
- People on long-term metformin. Metformin interferes with the calcium-dependent absorption of the B12–intrinsic factor complex, and the risk builds with duration — typically after several years of continuous use.
- People on long-term acid reducers. PPIs and H2 blockers work by suppressing the very stomach acid needed to free B12 from food. Taken alongside metformin, the risk appears higher still, particularly over 65.
- Vegans and strict vegetarians. B12 occurs naturally almost exclusively in animal foods; fortified cereals and nutritional yeast are among the few plant sources. Deficiency is notably more common in vegans who don't supplement.
- Pernicious anaemia. An autoimmune condition that destroys the stomach's ability to make intrinsic factor, the carrier B12 needs. It's the most common cause of B12 deficiency in the UK — and no amount of dietary change fixes it, which is precisely why it needs diagnosing rather than guessing.
- Gastric or bowel surgery, and gut conditions. Anything that removes or inflames the parts of the gut where B12 is released and absorbed can cause a shortfall years later.
Notice how many of those are things people are doing for good reasons, on their doctor's advice. This is not an argument against metformin or acid reducers — both treat real problems. It's an argument for knowing, so a fixable side issue doesn't get mistaken for something else.
What it feels like
Low B12 rarely announces itself. It creeps. The picture often includes memory slips and difficulty concentrating — what most people call brain fog — along with fatigue that sleep doesn't touch, tingling or numbness in the hands and feet (classically in a symmetrical "glove and stocking" pattern, often starting in the feet), and sometimes problems with balance or unsteadiness on your feet. Mood changes and a sore tongue can feature too.
Two things make this easy to dismiss. First, every one of those symptoms has a dozen other explanations, and "I'm just getting older" is the path of least resistance. Second, neurological symptoms can appear before anaemia shows on a routine blood count — so a normal full blood count is not the same as a normal B12. Worth knowing if a general blood test has already reassured you without specifically looking.
Why the guidelines say to check it
When a clinician evaluates someone with cognitive change, they don't start from Alzheimer's. They start by ruling out the things that can be fixed. Thyroid problems, depression, medication side effects, structural issues — and B12. The American Academy of Neurology's practice guidance has long recommended screening for B12 and folate deficiency and thyroid dysfunction as part of evaluating cognitive impairment, and that logic sits in cognitive workups internationally. The reasoning is refreshingly practical: the test is cheap, the treatment is cheap and safe, and the alternative — missing a correctable cause — is unacceptable.
Now the honesty this deserves. "Reversible cause" does not mean "dementia usually reverses." An often-cited meta-analysis by Clarfield (2003), pooling 39 studies and more than 5,000 people with dementia, found potentially reversible causes in around 9% of cases — but only about 0.6% actually reversed once treated. That number deserves to sit here rather than be quietly left out. The realistic framing: correcting low B12 is far more likely to help the milder end of the spectrum — the fog, the fatigue, the early slips — than to undo an established dementia. It's a box worth ticking, not a cure.

Here's where the hopeful story gets used to sell things it shouldn't. If deficiency causes fog, surely more B12 means a sharper brain? The trials say no. Studies of B vitamins in older adults who aren't deficient have been largely null: a 2025 meta-analysis in Nutrition Reviews, pooling 17 randomised trials and 5,275 participants, found only a very small effect on global cognition — and earlier reviews generally found no meaningful benefit at all, even though the supplements reliably did lower homocysteine as intended. Lowering the marker didn't deliver the memory. Tellingly, one analysis found folate-based B vitamins helped cognition in countries without mandatory folic-acid fortification but did essentially nothing where fortification was already in place — the benefit tracks the shortfall, not the pill.
So the shape of the evidence is consistent: fix a deficiency, maybe help; add B12 to sufficiency, expect nothing. It's the same pattern we found across the category when we looked at whether brain supplements work — the answer is almost always "only if you were missing something."
Which is why self-treating is the wrong move, even though B12 is cheap and low-risk. Taking it blindly can mask the problem without addressing the cause — pernicious anaemia needs identifying, not papering over — and it can muddy the very test that would have given you an answer. And if a deficiency is found, timing matters: B12-related nerve damage left untreated for a long time can become permanent, so it's a conversation worth having sooner.
One box ticked, then the longer game
Here's how to hold all of this at once. B12 belongs on the list of reversible causes worth ruling out — genuinely, and it's one blood test away. But once it's ruled out, or ruled in and treated, the bigger question remains: what actually shapes your risk over the decades? The 2024 Lancet Commission estimated that around 45% of dementia cases worldwide are associated with 14 modifiable risk factors — blood pressure, hearing, physical activity, blood sugar, smoking, sleep, mood, social connection. That's where the leverage lives, and none of it comes in a bottle — a verdict that has held every time a single compound has been tested properly, turmeric's curcumin included.
The B12 story is a good one precisely because it's honest in both directions: sometimes there's a simple, fixable answer worth looking for. And when there isn't, the unglamorous levers are still there, still working, still yours. Knowing which ones matter most for you is exactly what it means to check your dementia risk — and it rewards consistency far more than any single test result.
Can vitamin B12 deficiency cause memory loss or dementia?
